Tension Headache: Causes, Symptoms, and Diagnosis

A tension headache is one of the most common types of headaches, affecting a large portion of the global population. According to a recent study (Stovner, 2022), about 15,8% of the world’s population suffers of headaches daily, with a global prevalence of tension headaches estimated at 26%. Women are more affected than men, accounting for 30.8% of cases in women compared to 21.4% in men. In developed countries, it is estimated that one-third of men and half of women suffer from tension-type headaches. These headaches are often triggered by emotional factors such as stress and are considered one of the most common neurological conditions.
Symptoms
Generally, tension headaches are less intense and debilitating than migraines, but this varies from individual to individual, evidently. Unlike migraines, tension headaches are experienced as a sensation of pressure typicallyon both sides of the head or at the front, rather than a throbbing pain on one side.Theyare also less severe, ranging from mild to moderate intensity.
Here are the criteria for cluster headache as indicated by the ICHD-3:
At least 10 headache episodes occurring 1 to 14 days/month on average for more than 3 months (between ≥12 and less than 180 days/year) and meeting the following criteria:

Duration of 30 minutes to 7 days

At least two of the four following criteria :

location on both sides(bilateral)
-
pressing or tightening type (non-pulsatile)
-
mild to moderate intensity
-
no worsening with routine physical activities such as walking or climbing stairs

Presence of the two following criteria:

No nausea, no vomiting
-
no more than one of the two associated signs: photophobia or phonophobia

No other diagnostic better described in the ICHD-3
To have chronic headaches, the frequency must be more than 15 times per month on average for at least 3 months (or 180 days/year).
Causes
Unfortunately, the exact cause of tension headaches is not yet fully understood.. However, theories over the years suggest that tension headaches may be caused by various factors, including tension or contraction of the muscles in the neck and scalp in response to stress, depression, head injury, or anxiety. These muscle contractions can result from activities that maintain the head in a fixed position for a long period, such as working on a computer, performing intricate tasks with the hands, or using a microscope. Other triggers include physical or emotional stress, alcohol consumption, caffeine (excess or withdrawal), infections like the common cold, flu, or sinusitis, dental issues such as jaw clenching or teeth grinding, fatigue, excessive smoking, dry eyes, and eye strain from overuse.
Another popular theory suggests that individuals affected by tension headaches have a heightened sensitivity to pain compared to the average person. The muscle tension in the neck, often observed in people with tension headaches, may result from this sensitivity.
